Patients and methods: A randomized, double-blind, sham-controlled, multicenter study in 36 subjects with persistent low-back and/or radiating leg pain after lumbar surgery was conducted. Eligible subjects were randomized (1:1:1) to receive one of two doses of therapy (42-μs or 38-μs pulse width) or treatment with a sham device. Subjects self-treated twice daily for 60 days. The primary end point was change in pain intensity (∆PI) using the Numerical Pain Rating Scale between average baseline (Days −5 to −1) and end of treatment (Days 56–60) for lumbar and radiating leg pain. Secondary outcome measures included the Oswestry Disability Index, Beck Depression Inventory-II, Patient Global Impression of Change, and consumption of analgesics.

The development of modern PEMF has followed two separate pathways. The first pathway originated in more conventional (and still useful) electromagnetic field technologies broadly known as radio frequency (RF) diathermy.4 Continuous RF produces heat, the therapeutic component frequently employed in physical therapy. One early user of diathermy suspected that it could produce a nonthermal biologic effect.5 To test this idea clinically, the RF signal was intermittently pulsed, thereby eliminating heat. Positive outcomes, especially in treating inflammatory conditions, were reported.5 The first therapeutic RF PEMF device, the Diapulse, was commercialized in 19506 and was eventually cleared by the U.S. Food and Drug Administration (FDA) for the postoperative treatment of pain and edema in soft tissue. Studies emerged suggesting that PEMF could modulate the production of growth factors32 and began to focus on enzyme systems with well-characterized calcium (Ca2+) dependence. By the mid-1990s, researchers were investigating the effects of electrical33 and PEMF signaling on intracellular Ca2+, specifically the binding of Ca2+ to calmodulin (CaM), using the knowledge that CaM-dependent cascades were involved in tissue repair.34 One important early study showed that RF PEMF could increase Ca2+, binding kinetics to CaM by measuring the phosphorylation of myosin light chains in an enzyme assay.35 This and other studies36,37 clearly showed the dependence of the PEMF effect upon free Ca2+ at levels mimicking those found in the living cell. Therefore, PEMF modulates a physiologically relevant cascade involving Ca2+, binding to CaM. The Ca/CaM complex then binds to and activates myosin light chain kinase (MLCK), which in turn catalyzes myosin phosphorylation.38
Changes in blood pressure, skin electrical resistance, and pulse amplitude in 163 oncology patients exposed to tumor-specific PEMF frequencies have also been reported suggesting that PEMF therapy does not only target neoplastic cells, but may also have systemic effects. However, long-term PEMF treatment has been shown to be non-toxic, and employs 100,000 times lower frequencies as compared with radiofrequency ablation that is also employed for treatment of many cancers.

Taping patients may support retraining of correct movement patterns. However, using asymptomatic subjects Cools et al (2002) showed that tape application intended to inhibit the upper and facilitate the lower trapezius had no effect on electromyographic activity in the serratus anterior or all three portions of the trapezius with resisted or un-resisted flexion and abduction of the shoulder. The authors suggested altered timing as a possible explanation for the clinically observed effects of taping. In contrast, in patients with subacromial impingement Selkowitz et al (2007) did show that similar taping decreased upper trapezius and increased lower trapezius activity during a functional overhead-reaching task and that it decreased upper trapezius activity during shoulder abduction in the scapular plane. Mechanisms suggested to be involved in taping include facilitation or augmentation of proprioceptive cutaneous input, tension when movement occurs outside of the movement pattern allowed by the taping application, and inhibition or facilitation by taping shortened overactive muscles in a lengthened position, whereas the tape might be used hold lengthened under-active muscles in a shortened position. Various taping techniques appropriate for patients with impingement have been described in the literature (Morrissey 2000, Kneeshaw 2002) (Fig 16.5). Morrissey (2000) suggested that when the positive effect on the movement pattern or on symptoms was maintained, taping could be discontinued.
Dr. Pawluk also introduced a small, disposable PEMF device called Acti-Patch by Bioelectronics Corp. In Canada, ActiPatch is a Class II device and is sold over-the-counter. Health Canada has approved it for relief of musculoskeletal pain. It is not FDA-approved for musculoskeletal pain, so the efficacy of these patches is still unclear. Although I am certified in cardiology and gastroenterology, a significant portion of my practice is now devoted to exploring how PST achieves its benefits.10 I have treated 1000 patients, most of whom had knee OA, with very gratifying results similar to those reported in the literature. In my opinion this means that the regulations are entirely inadequate. Alzheimer’s disease: improvement of visual memory and visuoconstructive performance by treatment with picotesla range magnetic fields; Sandyk R; Int J Neurosci. 1994 Jun;76(3-4):185-225. The author had previously reported that external application of electromagnetic fields (EMF) of extremely low intensity (in the picotesla range) and of low frequency (in the range of 5Hz-8Hz) improved visual memory and visuoperceptive functions in patients with Parkinson's disease. Since a subgroup of Parkinsonian patients, specifically those with dementia, have coexisting pathological and clinical features of AD, the author investigated in two AD patients the effects of these extremely weak EMF on visual memory and visuoconstructive performance. The Rey-Osterrieth Complex Figure Test as well as sequential drawings from memory of a house, a bicycle, and a man were employed to evaluate the effects of EMF on visual memory and visuoconstructive functions, respectively. After plugging in your mat, turn up the temperature to the maximum setting for 10 minutes to allow the mat to warm up.
