In aging and debilitated cells, the inflow of nutrients accompanied is also accompanied with a current of toxic fluids. It has been established that pain is felt as a result of the stimulation of peripheral nociceptors. When this stimulation occurs, encoded signals of distress are transmitted in an ascending pattern to the uppermost cortical structures.

Exercise therapy interventions for patients with impingement are intended to restore the frontal and transverse plane glenohumeral force couples and normalize scapular motion. Generally they consist of progressive resistive exercises for the rotator cuff and scapular muscles and stretching of tight structures but they should also address the motor control deficits identified in patients with impingement. More detail on shoulder exercises is provided in Chapters 21 and 22. Exercise interventions have been supported in a number of recent randomized trials (Werner et al 2002, Walther et al 2004, Lombardi et al 2008) and systematic literature reviews for producing improvements in both pain and function (Green et al 2003, Trampas & Kitsios 2006, Faber et al 2006). In a Cochrane review (Green et al 2003), exercise was noted as effective in terms of short-term recovery in rotator cuff disease (RR 7.74; 95% CI 1.97–30.32) and for longer-term benefit with regard to function (RR 2.45; 95% CI 1.24–4.86). It should be noted that in patients with Neer stage I–II impingement there are no significant between-group differences (at 6 and 12 weeks) with regard to pain and function for patients treated with a supervised exercise programme or a home programme in which they are instructed by a physical therapist (Werner et al 2002, Walther et al 2004).

The primary end point was change in pain intensity (∆PI) using the Numerical Pain Rating Scale between average baseline (Days −5 to −1) and end of treatment (Days 56–60) for lumbar and radiating leg pain. Secondary outcome measures included the Oswestry Disability Index, Beck Depression Inventory-II, Patient Global Impression of Change, and consumption of analgesics.
The basic science work accomplished to date provides strong support for the proposal that modulation of Ca2+ binding to CaM, upon a transient increase in intracellular calcium when homeostasis is interrupted,48 is an important PEMF transduction pathway. It is likely that the disruption of the tightly regulated Ca2+ balance in cells is the natural signal to provoke the endogenous tissue repair and regeneration mechanism, hence the apparent simple acceleration of normal healing activity by targeted PEMF signals. Ca/CaM catalyzes eNOS, which allows the PEMF signal to modulate the release of NO from eNOS and potentially affect the entire tissue repair pathway, from pain and edema to angiogenesis, bone and tissue regeneration, and other regenerative actions. PEMF signals configured to target the Ca/CaM pathway have been applied to rat tendon and wound healing.49,50 In both studies, tendon and wound healing rates were seen to significantly increase by 59% ± 4% (Figure 1, A) and 69% ± 5% (Figure 1, B) in PEMF-treated animals.

Bone regeneration can be accelerated in osteotomies made 3.5–4.5 cm distal to the head of the fibula in dogs. A voltage field was induced in the fibula by inductively coupling pulsed electromagnetic fields of low frequency and strength directly to the bone across the skin. Not only did the stimulated osteotomies heal faster than control osteotomies but the regenerated bone was also more highly organized and stronger than control regenerated bone, even though the mass of callus formed was less than in controls. This method has been used to successfully treat tibial pseudoarthroses in young patients. Pseudoarthrosis is a rare, local bone dysplasia that has a very low probability of correction by conventional techniques. Sixty patients wore either a real PEMF device for 12 hours per day, or a fake; neither they nor the researchers knew who got real PEMF (double-blind). PEMF is particularly easy to test properly, because it causes no sensation, making it much easier to compare to an active placebo.

Several mechanisms of PEMF therapy have been elucidated with regards to cancer. These studies have shown that PEMF therapy may exert proliferative inhibition and mitotic spindle disruption, block the development of neovascularization (blood supply) required for tumor growth and exacerbate an inherent or induced genetic instability by reducing the stringency of the late-cycle (G2) checkpoint. PEMF therapy also modulates gene expression and protein synthesis, interacting with specific DNA sequences within gene promoter regions. PEMFs have also an immunomodulatory effect, as supported by in vivo evidence showing an increase in tumor necrosis factor alpha levels that induce an anti-tumoral response.

Separate Cochrane Systematic Reviews evaluating the benefits of electrotherapy modalities as well as assessing the value of manual therapy and exercise for rotator cuff disease have been performed (Page, Green, McBain, et al, 2016; Page, Green, Mrocki, et al, 2016). Modalities such as transcutaneous electric nerve stimulation, therapeutic ultrasound, low-level laser therapy (LLLT), and pulsed electromagnetic field therapy are examples of the modalities potentially utilized in the electrotherapy evaluation. On the basis of low-quality evidence, therapeutic ultrasound may have short-term benefits for patients with calcific tendinitis, and LLLT may have short-term benefits in patients with rotator cuff disease. The review of the literature identified only 1 of 60 trials to be of high quality regarding manual therapy and exercise, and no benefit was noted.
Self-administered PEMF therapy for 60 min, three times a day, for an average of 278.4 months was offered to only 28 patients with advanced cancer: 7 breast cancer, 5 ovarian cancer, 3 pancreatic cancer, 2 colorectal cancer, 2 prostate cancer, 1 glioblastoma multiforme, 1 hepatocellular carcinoma, 1 mesothelioma, 1 neuroendocrine tumor, 1 non-small-cell lung cancer, 1 oligodendroglioma, 1 small-cell lung cancer, 1 sarcoma, and 1 thyroid tumor. None of the patients who received PEMF therapy reported any side effects; four patients presented stable disease for 3 years (thyroid cancer with biopsy-proven lung metastases), 6 months (mesothelioma metastatic to the abdomen), 5 months (non-small-cell lung cancer), and 4 months (pancreatic cancer with biopsy-proven liver metastases), respectively.

Considered together, all available evidence provides strong support for Ca/CaM-dependent transient NO production as an important PEMF transduction pathway for tissue repair. One of the authors (AAP) has recently proposed the PEMF mechanism as a working model for PEMF therapeutics (Figure 4).61 This mechanism suggests that the primary effect of clinically relevant PEMF signals is to increase the rate of Ca2+ binding to CaM, which then catalyzes cNOS (eg., eNOS), producing an immediate (within seconds) production of NO, which can orchestrate an antiinflammatory response via increased blood and lymph flow. NO, in turn, regulates cGMP production (within minutes), which cascades to the appropriate growth factor release dependent on the stage of healing (eg., FGF-2 for angiogenesis).
