Pulsed electromagnetic field therapy was approved by the FDA in 1979 specifically for the healing of nonunion fractures, which came after a Columbia University study that was encouraged by NASA, and has recently gained attention in the U.S (even appearing as a segment on the Dr. Oz Show). The value of pulsed electromagnetic field therapy has been shown to cover a wide range of conditions, with well documented trials carried out by hospitals, rheumatologists, physiotherapists, and neurologists.
So now I can return to the question I posed at the beginning of this article, “Is Pulsed Electromagnetic Field Therapy dangerous?” In short, No. From all the evidence I have seen and read, there seems to be no immediate damage caused by PEMF. If in fact, this is just an artificially recreated version of the Earths own Pulsed Electromagnetic Field, then I can understand why we aren’t being damaged by it; after all we did evolve under its effects.
Classically, TENS has been used for the purpose of pain alleviation. Low-frequency TENS has also been found effective to increase microcirculation and facilitate the absorption of calcific deposits in the shoulder tendons.123,124 The most effective treatment points are believed to be associated with stimulation of the acupuncture points.125 Figure 41-9 displays a possible electrode placement using acupuncture sites. The points used in this arrangement include Jianjing (GB 21), Binao (LI 14), Juga (LI 16), and Jianya (LI 15).126 Any physical modality is only an adjunct in a physical therapy clinic and should be used with prudence.
Widely used in Europe, and growing in popularity across North America, pulsed electromagnetic field therapy has been used for decades by doctors, chiropractors, naturopaths and even veterinarians. In the past, PEMF devices were quite large and prohibitively expensive for most of the general public. Today, however, PEMF devices are used in many homes, and costs have fallen considerably.
After plugging in your mat, turn up the temperature to the maximum setting for 10 minutes to allow the mat to warm up. Doing this allows the copper coils in the mat to become fully heated, releasing more PEMF waves. After 10 minutes, reduce the temperature to a desired temperature and once the mat reaches that temperature, lie down on the PEMF mat and begin your mat session.
All energy is electromagnetic in nature. All atoms, chemicals and cells produce electromagnetic fields (EMFs). Every organ in the body produces its own signature bio-electromagnetic field. Although I am certified in cardiology and gastroenterology, a significant portion of my practice is now devoted to exploring how PST achieves its benefits.10 I have treated 1000 patients, most of whom had knee OA, with very gratifying results similar to those reported in the literature. The panel cited two prior efforts to establish guidelines for treating knee OA, emphasising that these “primarily represent consensus statements from expert panels” and “The type and strength of evidence to support such guidelines remain unclear.” Their stated objective, therefore, was to “develop guidelines relating to clinical issues in OA management, and to indicate clearly the level of evidence to support individual statements”. However, electromagnetic therapy approaches were again omitted, though at least one of the members is quite familiar with PST. The reason for this exclusion is not clear and I believe that your readership deserves to be aware of this extremely safe and effective option.
Senbursa et al (2007) compared a home programme of rotator cuff and scapular strengthening exercises, active range of motion, and stretching with 12 sessions of glenohumeral soft tissue and joint mobilization, ice application, stretching and strengthening exercises in patients with impingement. At 4 weeks there were significant between group differences with regard to pain and function favouring the manual therapy group. Kachingwe et al (2008) showed significant changes with regard to pain, pain-free range of motion, and function for patients with impingement treated with 6 sessions of supervised exercise only, supervised exercise with glenohumeral grade I–IV glide and traction mobilizations from midrange, supervised exercise with a Mulligan mobilization with movement (MWM) shoulder flexion technique, or a control group receiving only physician advice; there were no between-group differences. Although power in this pilot study was extremely limited, the three intervention groups had a greater improvement in function and both manual therapy groups improved more with regard to pain measures. Active range of motion increased most for the MWM and least for the mobilization group.

This was a rigorous test of wearable pulsed electromagnetic fields (PEMF) for older patients with osteoarthritis of the knee: moderate to severe cases with X-ray evidence and pain of at least 4/10 for more than six months, despite maximum tolerated medication. Sixty patients wore either a real PEMF device for 12 hours per day, or a fake; neither they nor the researchers knew who got real PEMF (double-blind). PEMF is particularly easy to test properly, because it causes no sensation, making it much easier to compare to an active placebo.
Pulsed electromagnetic field therapy, with other less commonly used interventions, was not included in this list, and its evidence for efficacy was therefore not assessed. However, as emphasised by Dr Pfeiffer, an evidence based evaluation of all other interventions would be of interest and could be considered for inclusion in the next round of evidence based guidelines.

RESULTS: Sixty-six patients were included, and 60 completed the study. After 1 month, PEMF induced a significant reduction in VAS pain and WOMAC scores compared with placebo. Additionally, pain tolerance, as expressed by PPT changes, and physical health improved in PEMF-treated patients. A mean treatment effect of -0.73 (95% CI - 1.24 to - 0.19) was seen in VAS score, while the effect size was -0.34 (95% CI - 0.85 to 0.17) for WOMAC score. Twenty-six per cent of patients in the PEMF group stopped NSAID/analgesic therapy. No adverse events were detected.

Studies emerged suggesting that PEMF could modulate the production of growth factors32 and began to focus on enzyme systems with well-characterized calcium (Ca2+) dependence. By the mid-1990s, researchers were investigating the effects of electrical33 and PEMF signaling on intracellular Ca2+, specifically the binding of Ca2+ to calmodulin (CaM), using the knowledge that CaM-dependent cascades were involved in tissue repair.34 One important early study showed that RF PEMF could increase Ca2+, binding kinetics to CaM by measuring the phosphorylation of myosin light chains in an enzyme assay.35 This and other studies36,37 clearly showed the dependence of the PEMF effect upon free Ca2+ at levels mimicking those found in the living cell. Therefore, PEMF modulates a physiologically relevant cascade involving Ca2+, binding to CaM. The Ca/CaM complex then binds to and activates myosin light chain kinase (MLCK), which in turn catalyzes myosin phosphorylation.38
