Within the domain of shoulder pain, rotator cuff conditions can be caused by an inter-relationship between soft tissue laxity (i.e. ligament) resulting in glenohumeral laxity, impingement (e.g. due to bursitis or osteophytes) resulting in tendon compression and cuff lesions (Allingham & McConnell 2003). Therefore, treatment is likely to be more effective when all possible factors that can cause laxity, impingement or lesion of the cuff are considered. These include:
Exposure to a specific pulsed electromagnetic field (PEMF) has been shown to produce analgesic (antinociceptive) effects in many organisms. In a randomized, double-blind, sham-controlled clinical trial, patients with either chronic generalized pain from fibromyalgia (FM) or chronic localized musculoskeletal or inflammatory pain were exposed to a PEMF (400 μT) through a portable device fitted to their head during twice-daily 40 min treatments over seven days. The effect of this PEMF on pain reduction was recorded using a visual analogue scale. A differential effect of PEMF over sham treatment was noticed in patients with FM, which approached statistical significance (P=0.06) despite low numbers (n=17); this effect was not evident in those without FM (P=0.93; n=15). PEMF may be a novel, safe and effective therapeutic tool for use in at least certain subsets of patients with chronic, nonmalignant pain.
"PEMF therapy applied to a 49-year-old male PD patient with stage 3 disease, as assessed by Hoehn and Yahr scale, resulted in a marked improvement in motor and non-motor symptoms such as mood swings, sleeplessness, pain and sexual and cognitive dysfunctions, suggesting that PEMF therapy should be tested in large cohorts of PD patients as monotherapy and should also be considered as a treatment modality for de novo diagnosed PD patients. PEMF therapy was also effective in improving visuospatial deficits in four PD patients. Moreover, PEMF therapy improved PD-associated freezing (a symptom manifesting as a sudden attack of immobility usually experienced during walking) in 3 PD patients".
The feasibility of PEMF therapy for treatment of hepatocellular carcinoma (HCC) has also been investigated in a single-group, open-label, phase I/II clinical study. Forty-one patients with advanced HCC received very low levels of PEMFs modulated at HCC-specific frequencies (100 Hz–21 kHz) and received three-daily 60 min outpatient treatments. No adverse reactions were observed during PEMF treatment. Five patients reported complete disappearance and two patients reported decrease in pain shortly after beginning of treatment. Four patients showed a partial response to treatment, while 16 patients (39%) had stable disease for more than 12 weeks. This study shows that PEMF therapy provides a safe and well-tolerated treatment, as well as evidence of antineoplastic effects in patients with HCC.

It is important to note that PEMF effects are immediate and are not limited by pharmacokinetics because the induced currents are instantaneously present when the coil applicator is transmitting into the affected area. For example, studies designed to assess PEMF effects on pain and edema in a carrageenan rat hind paw model have reported a 100% inhibition of pain and a 50% reduction of edema in treated animals over a time span of 225 minutes62 compared with aspirin or nitroaspirin, which only caused about 50% pain inhibition at 200 minutes, using maximum dose in the same model.63 It is also important to note that resting cells (in homeostasis), in which there is no transient increase in cytosolic free Ca2+, do not appear to respond to PEMF, providing one explanation for the reports of no known side effects from PEMFs since the clearance of BGS devices in 1979.

Although cross-sectional comparison of different shoulder questionnaires can show comparable overall validity and patient acceptability, it is important to include overhead activities since overhead work is an important aspect of her daily work. An additional benefit of the SPADI is that it is responsive to change, quick to complete, and scores are not likely to change in stable subjects (Paul et al 2004).

Corticosteroid injections have been commonly utilized in treating rotator cuff disease. Koester et al (2007) performed a systematic review of the literature and analyzed nine randomized controlled studies comparing subacromial corticosteroid injection with placebo. One study demonstrated significant pain relief and two studies showed an increased range of motion in the injection group. No significant complications were identified. In a study comparing a corticosteroid injection with a platelet-rich plasma (PRP) injection for subacromial impingement syndrome, the investigators found the Constant score and VAS for pain to be significantly better at both 6 weeks and 6 months in the corticosteroid-treated group. Good patient candidates for a single subacromial corticosteroid injection to achieve pain control are those with significant night pain or patients who will not tolerate phase 1 rehabilitation because of pain.

Bergman et al (2004) compared medical care (consisting of oral analgesics or NSAID, education, advice, corticosteroid infiltrations and physical therapy referral for exercise, modalities, massage after 6 weeks) to medical care with up to 6 treatments of thrust and non-thrust manipulative interventions to the ribs and cervical-thoracic spine over 12 weeks in patients with shoulder symptoms and dysfunction of cervico-thoracic spine and adjacent ribs. At 12 weeks, 43% of the manipulation group and 21% of the medical care group reported full recovery. A 17-percentage point difference favouring manipulation still existed at 52 weeks. Pulsed Electromagnetic Field Therapy (PEMF) uses electrical and magnetic fields to address a variety of health issues. Nonetheless, a multitude of studies have shown the BGS signal to have sufficient biologic effect to modulate growth factor release.25 Therapeutic uses of these technologies in orthopedics have led to clinical applications approved by regulatory bodies worldwide for the adjunctive treatment of recalcitrant fractures and spine fusion.1,26 Several reports have suggested that the overall success rate of BGS is not significantly different from that of the first bone graft,1 which is a significant benefit for the patient and the health care system.

The development of modern PEMF has followed two separate pathways. The first pathway originated in more conventional (and still useful) electromagnetic field technologies broadly known as radio frequency (RF) diathermy.4 Continuous RF produces heat, the therapeutic component frequently employed in physical therapy. One early user of diathermy suspected that it could produce a nonthermal biologic effect.5 To test this idea clinically, the RF signal was intermittently pulsed, thereby eliminating heat. Positive outcomes, especially in treating inflammatory conditions, were reported.5 The first therapeutic RF PEMF device, the Diapulse, was commercialized in 19506 and was eventually cleared by the U.S. Food and Drug Administration (FDA) for the postoperative treatment of pain and edema in soft tissue. Clinical devices in use since that time typically have consisted of a large signal generator and a bulky coil applicator positioned over the area of injury that delivers therapy noninvasively, through either dressings or clothing. Early devices were expensive, nonportable, and produced significant electromagnetic interference (EMI); these factors restricted more widespread use in outpatient and home settings.

Ultrasound refers to a high-frequency nonaudible acoustic energy that travels in the form of longitudinal mechanical waves. Traditionally used by physical therapists to intervene in injuries to soft tissues, it is most commonly used with intensity in the range of 0.5–2.0 W/cm2. In comparison, to intervene in injuries to hard tissues (such as bone) pulsed-wave ultrasound with a spatially averaged, temporally averaged intensity (ISATA) of below 0.1 W/cm2 is preferred. ISATA refers to the average ultrasound power over the area of the ultrasound beam (spatial-average) and the average of this intensity over a complete pulse cycle (ultrasound ‘on’ and ‘off’ period; temporal average). Pulsed-wave ultrasound with an ISATA below 0.1 W/cm2 is termed LIPUS and is preferred in the intervention of fracture healing, as its low ISATA significantly reduces the risk of any thermal or cavitational tissue damage—LIPUS has US Food and Drug Administration approval to be applied to bone.

The effect of this PEMF on pain reduction was recorded using a visual analogue scale. A differential effect of PEMF over sham treatment was noticed in patients with FM, which approached statistical significance (P=0.06) despite low numbers (n=17); this effect was not evident in those without FM (P=0.93; n=15). Low-intensity pulsed ultrasound (LIPUS) has been shown to have significant beneficial skeletal effects. Ultrasound refers to a high-frequency nonaudible acoustic energy that travels in the form of longitudinal mechanical waves. Traditionally used by physical therapists to intervene in injuries to soft tissues, it is most commonly used with intensity in the range of 0.5–2.0 W/cm2. In comparison, to intervene in injuries to hard tissues (such as bone) pulsed-wave ultrasound with a spatially averaged, temporally averaged intensity (ISATA) of below 0.1 W/cm2 is preferred. ISATA refers to the average ultrasound power over the area of the ultrasound beam (spatial-average) and the average of this intensity over a complete pulse cycle (ultrasound ‘on’ and ‘off’ period; temporal average). Pulsed-wave ultrasound with an ISATA below 0.1 W/cm2 is termed LIPUS and is preferred in the intervention of fracture healing, as its low ISATA significantly reduces the risk of any thermal or cavitational tissue damage—LIPUS has US Food and Drug Administration approval to be applied to bone.
