The gentlemen I am looking into gut health for has found after a couple of our sessions, but not all the time, he is able to play piano after 9 years of his tremor being too disruptive. ranscutaneous electrical nerve stimulation (TENS or TNS) is the use of electric current produced by a device to stimulate the nerves for therapeutic purposes. One well-designed study clearly showed DNA synthesis in articular chondrocytes in vitro was increased by PEMF via a NO pathway. This study systematically used CaM, NOS, and cGMP inhibitors that individually eliminated the PEMF effect on DNA synthesis.55 PEMF effects on osteoblast proliferation and differentiation were shown to be mediated by NO.56 Direct evidence of the effect of a PEMF signal configured for the Ca/CaM pathway on real-time NO production in a neuronal cell line, which could be eliminated by CaM and NOS inhibitors, has also recently been reported.57,58 Finally, 2 of the authors (BS and AAP) showed that PEMFs configured for the NO pathway significantly increased angiogenesis in a thermal myocardial injury in the rat.59,60 This effect was eliminated in rats who were fed NG-nitro-L-arginine methyl ester (L-NAME), a NOS inhibitor (Figure 3).

The research to date has shown that the mechanisms by which PEMF works are complicated and likely involve many pathways. In addition to increasing cell metabolism, perhaps PEMF’s greatest power is in its ability to ameliorate the effects of inflammation by decreasing inflammatory cytokines. it is also conceivable, as suggested by Gordon et al, that another important effect of PEMF is the ability of the magnetic fields to restore “equilibrium in ROS (free radical)/antioxidant chemistry. It is unequivocal that all chronic diseases result from a lack of homeostasis between free radicals and antioxidants. While both free radicals and antioxidants are normal and vital for processes such as cellular respiration and immunity, an imbalance could lead to cell and tissue death, DNA damage, and protein and fat degradation.
In a parallel but separate evolution, orthopedic surgeons, studying the process by which mechanical signals influence bone growth and repair,16,17 discovered that everyday mechanical signals (walking, jumping, etc.) produced endogenous electrical currents in bone that could modulate bone cell activity.18 This naturally led to the use of exogenous current for bone repair. The first animal studies employed low (microampere) level direct currents (DC) delivered via implanted electrodes that resulted in new bone formation around the negative electrode (cathode19). The first therapeutic devices were based on these early animal studies and used implanted and semi-invasive electrodes delivering DC to the fracture site.20,21 These early applications required the cathode to be near the fracture site because bone growth was limited to the area immediately adjacent to the electrode surface, where chemical changes related to electrolysis occur.
Separate Cochrane Systematic Reviews evaluating the benefits of electrotherapy modalities as well as assessing the value of manual therapy and exercise for rotator cuff disease have been performed (Page, Green, McBain, et al, 2016; Page, Green, Mrocki, et al, 2016). Modalities such as transcutaneous electric nerve stimulation, therapeutic ultrasound, low-level laser therapy (LLLT), and pulsed electromagnetic field therapy are examples of the modalities potentially utilized in the electrotherapy evaluation. On the basis of low-quality evidence, therapeutic ultrasound may have short-term benefits for patients with calcific tendinitis, and LLLT may have short-term benefits in patients with rotator cuff disease. The review of the literature identified only 1 of 60 trials to be of high quality regarding manual therapy and exercise, and no benefit was noted.

At the same time, we anticipate that improved signals and products that are more effective and more ergonomically designed will be developed, and that other electrochemical pathways will be targeted for additional indications. This may finally be the century of electrotherapy. In the meantime, plastic surgeons have at hand a powerful tool for the adjunctive management of postoperative pain and edema and wound repair. PEMF therapy is simple, cost-effective, has no known side effects, and may well play a large role in treatment of otherwise intractable wounds while reducing the cost of health care.

Corticosteroid injections have been commonly utilized in treating rotator cuff disease. Koester et al (2007) performed a systematic review of the literature and analyzed nine randomized controlled studies comparing subacromial corticosteroid injection with placebo. One study demonstrated significant pain relief and two studies showed an increased range of motion in the injection group. No significant complications were identified. In a study comparing a corticosteroid injection with a platelet-rich plasma (PRP) injection for subacromial impingement syndrome, the investigators found the Constant score and VAS for pain to be significantly better at both 6 weeks and 6 months in the corticosteroid-treated group. Good patient candidates for a single subacromial corticosteroid injection to achieve pain control are those with significant night pain or patients who will not tolerate phase 1 rehabilitation because of pain.

My experience as a physio in the community has involved lots of work with Parkinson's and increasingly I see the effects of stress and the social engagement system being critical to understanding and improving movement, and in the last 3 years have done much more work with body awareness during activity, whether it be gaining flexibility or strength or balance. The toughest part is engagement especially when the general physio community is not promoting the same message. As you're website implies, it requires such a commitment to your well-being. I totally admire your perseverance and have empathy for how challenging it must be for you at times.
