Although the aforementioned studies suggest that PEMF effects on the Ca/CaM/NO cascades were responsible for the reported biologic effects, cellular and animal studies in which the use of inhibitors for selected steps in this cascade provide the strongest support. One well-designed study clearly showed DNA synthesis in articular chondrocytes in vitro was increased by PEMF via a NO pathway. This study systematically used CaM, NOS, and cGMP inhibitors that individually eliminated the PEMF effect on DNA synthesis.55 PEMF effects on osteoblast proliferation and differentiation were shown to be mediated by NO.56 Direct evidence of the effect of a PEMF signal configured for the Ca/CaM pathway on real-time NO production in a neuronal cell line, which could be eliminated by CaM and NOS inhibitors, has also recently been reported.57,58 Finally, 2 of the authors (BS and AAP) showed that PEMFs configured for the NO pathway significantly increased angiogenesis in a thermal myocardial injury in the rat.59,60 This effect was eliminated in rats who were fed NG-nitro-L-arginine methyl ester (L-NAME), a NOS inhibitor (Figure 3).

PEMFs are frequency-based, applied to either the whole body or parts of the body.  PEMFs may only be needed for short periods of time, while the effects last for many hours, setting in motion cellular and whole-body changes to restore and maintain balance in metabolism and health. The body does not acclimate, or “get used to,” the healthy energy signals of therapeutic PEMFs, even if used for a long time, compared to magnets.

Stationary (or “static”), non-varying, magnetic fields from magnets have fixed strengths. They are used in mattresses, bracelets, knee wraps and the like. The majority of these reactions are temporary and mild. In people with electromagnetic hypersensitivity and electrical hypersensitivity, these reactions are not only increasingly common but also more uncomfortable, yet magnetic therapy is seldom discontinued in this situation. The original PEMF devices consisted of a Helmholtz coil which generated a magnetic field. The patient’s body was placed inside the magnetic field to deliver treatment. Today, the majority of PEMF wellness devices resemble a typical yoga mat in dimensions but are slightly thicker to house several flat spiral coils to produce an even electromagnetic field. A frequency generator is then used to energize the coils to create a “pulsed” electromagnetic field.
Some systematic reviews (Green et al 2003, Faber et al 2006) have supported a combination of manual therapy and exercise for patients with impingement for improvements in pain and function. METHODS: In this randomized [with equal randomization (1:1)], double-blind, placebo-controlled clinical trial, patients with radiographic evidence of knee OA and persistent pain higher than 40 mm on the visual analog scale (VAS) were recruited. The trial consisted of 12 h daily treatment for 1 month in 60 knee OA patients. The primary outcome measure was the reduction in pain intensity, assessed through VAS and WOMAC scores. Secondary outcomes included quality of life assessment through the 36-item Medical Outcomes Study Short-Form version 2 (SF-36 v2), pressure pain threshold (PPT) and changes in intake of NSAIDs/analgesics.
Recently, I've started using it consistently for longer times, keeping it on through several cycles of the PD drugs. In doing so, I've found that this can reduce the prolonged "off" periods in which the drugs wear off completely. Thus, consistent use of the device is helping to keep pain and rigidity symptoms at bay, and to also extend the time of action of each dose too, allowing a lower drug burden overall. I also therefore get less, and less severe, dyskinesia - the large scale uncontrolled wriggling movements which long term use of the PD drugs tends to cause (side-effect of medication).

Exercise therapy interventions for patients with impingement are intended to restore the frontal and transverse plane glenohumeral force couples and normalize scapular motion. Generally they consist of progressive resistive exercises for the rotator cuff and scapular muscles and stretching of tight structures but they should also address the motor control deficits identified in patients with impingement. More detail on shoulder exercises is provided in Chapters 21 and 22. Exercise interventions have been supported in a number of recent randomized trials (Werner et al 2002, Walther et al 2004, Lombardi et al 2008) and systematic literature reviews for producing improvements in both pain and function (Green et al 2003, Trampas & Kitsios 2006, Faber et al 2006). In a Cochrane review (Green et al 2003), exercise was noted as effective in terms of short-term recovery in rotator cuff disease (RR 7.74; 95% CI 1.97–30.32) and for longer-term benefit with regard to function (RR 2.45; 95% CI 1.24–4.86). It should be noted that in patients with Neer stage I–II impingement there are no significant between-group differences (at 6 and 12 weeks) with regard to pain and function for patients treated with a supervised exercise programme or a home programme in which they are instructed by a physical therapist (Werner et al 2002, Walther et al 2004).
Although I am certified in cardiology and gastroenterology, a significant portion of my practice is now devoted to exploring how PST achieves its benefits.10 I have treated 1000 patients, most of whom had knee OA, with very gratifying results similar to those reported in the literature. The panel cited two prior efforts to establish guidelines for treating knee OA, emphasising that these “primarily represent consensus statements from expert panels” and “The type and strength of evidence to support such guidelines remain unclear.” Their stated objective, therefore, was to “develop guidelines relating to clinical issues in OA management, and to indicate clearly the level of evidence to support individual statements”. However, electromagnetic therapy approaches were again omitted, though at least one of the members is quite familiar with PST. The reason for this exclusion is not clear and I believe that your readership deserves to be aware of this extremely safe and effective option.

Considered together, all available evidence provides strong support for Ca/CaM-dependent transient NO production as an important PEMF transduction pathway for tissue repair. One of the authors (AAP) has recently proposed the PEMF mechanism as a working model for PEMF therapeutics (Figure 4).61 This mechanism suggests that the primary effect of clinically relevant PEMF signals is to increase the rate of Ca2+ binding to CaM, which then catalyzes cNOS (eg., eNOS), producing an immediate (within seconds) production of NO, which can orchestrate an antiinflammatory response via increased blood and lymph flow. NO, in turn, regulates cGMP production (within minutes), which cascades to the appropriate growth factor release dependent on the stage of healing (eg., FGF-2 for angiogenesis).

LIPUS stimulates bone union. The initial benefit of LIPUS on the skeleton in vivo is the induction of bone repair in fractures displaying either delayed union or nonunion. In a fracture nonunion model in rodents, 6 weeks of LIPUS treatment stimulated union in 50% of fractures. In a randomized, double-blind, sham-controlled clinical trial, patients with either chronic generalized pain from fibromyalgia (FM) or chronic localized musculoskeletal or inflammatory pain were exposed to a PEMF (400 μT) through a portable device fitted to their head during twice-daily 40 min treatments over seven days. The effect of this PEMF on pain reduction was recorded using a visual analogue scale. A differential effect of PEMF over sham treatment was noticed in patients with FM, which approached statistical significance (P=0.06) despite low numbers (n=17); this effect was not evident in those without FM (P=0.93; n=15). PEMF may be a novel, safe and effective therapeutic tool for use in at least certain subsets of patients with chronic, nonmalignant pain.

Successful studies typically use frequencies between 1-100 hz and with some very specific applications up to 1000 Hz have been used. Our Earth's natural environment and occurring magnetic field's though is what we humans have evolved with and we require. This is the best indicator of suitable PEMF frequency to bring back the body into it's natural state. The primary Schumann resonance would have the most effect although the other resonance points would also have an effect.
Having healthy cells is not a passive process. Active, regular tuning-up of our cells is not only feasible, but also necessary to slow aging and reduce the risk of cell dysfunction. CONCLUSIONS: This review shows that plastic surgeons have at hand a powerful tool with no known side effects for the adjunctive, noninvasive, nonpharmacologic management of postoperative pain and edema. Given the recent rapid advances in development of portable and economical PEMF devices, what has been of most significance to the plastic surgeon is the laboratory and clinical confirmation of decreased pain and swelling following injury or surgery.
Corticosteroid injections have been commonly utilized in treating rotator cuff disease. Koester et al (2007) performed a systematic review of the literature and analyzed nine randomized controlled studies comparing subacromial corticosteroid injection with placebo. One study demonstrated significant pain relief and two studies showed an increased range of motion in the injection group. No significant complications were identified. In a study comparing a corticosteroid injection with a platelet-rich plasma (PRP) injection for subacromial impingement syndrome, the investigators found the Constant score and VAS for pain to be significantly better at both 6 weeks and 6 months in the corticosteroid-treated group. Good patient candidates for a single subacromial corticosteroid injection to achieve pain control are those with significant night pain or patients who will not tolerate phase 1 rehabilitation because of pain.

Laser therapy was not demonstrated to be superior to placebo for patients with rotator cuff tendinopathy (Green et al 2003). Ultrasound (RR 1.81, 95% CI 1.26–2.60) and pulsed electromagnetic field therapy (RR 19, 95% CI 1.16–12.43) resulted in improvement compared to placebo with regard to pain in patients with calcific tendinopathy. There is no evidence of an effect for ultrasound in patients with other tendinopathy. Ultrasound also provides no additional benefit when used in combination with exercise interventions over exercise alone (Green et al 2003). There is strong evidence that extra-corporeal shock-wave therapy is no more effective than placebo in patients with impingement with regard to functional limitations (Faber et al 2006).

PEMFs have been in clinical use for generations. For most of that time, however, PEMFs have been relegated to second or even third tier status, with some ardent supporters, a number of skeptics, and most clinicians and patients simply unaware of their benefits. As this body of evidence grows and clinical experience widens, the gaps in the current knowledge (especially concerning optimal treatment regimens for specific conditions) will be filled. At the same time, we anticipate that improved signals and products that are more effective and more ergonomically designed will be developed, and that other electrochemical pathways will be targeted for additional indications. This may finally be the century of electrotherapy. In the meantime, plastic surgeons have at hand a powerful tool for the adjunctive management of postoperative pain and edema and wound repair. PEMF therapy is simple, cost-effective, has no known side effects, and may well play a large role in treatment of otherwise intractable wounds while reducing the cost of health care.
Magnetic therapy is simply the therapeutic application of magnets and can be delivered in many forms. In a 12-week, randomized trial, subjects with MS laid down on a metal mat for 8 minutes twice daily. The device delivered low-frequency, pulsed electromagnetic field therapy. Another study had subjects with MS wear wristwatch-size, magnetic pulsing devices called Enermed for 10 to 24 hours daily for 2 months. Another device also delivered low-frequency magnetic stimulations at 37.5 mT and a sequence of pulses at 4 to 7 Hz three times weekly for 2 months. These studies demonstrated consistent benefits in reducing fatigue but no benefit for depression in subjects with MS.53

Pulsed electromagnetic field therapy replicates the Earth’s natural magnetic field, which we were once strongly connected to. Over time this magnetic field has gotten weaker (an estimated 10% weaker since the 19th century) and due to industrialization and modernization, we are further separated from the Earth’s natural magnetic field.
