Corticosteroid injections have been commonly utilized in treating rotator cuff disease. Koester et al (2007) performed a systematic review of the literature and analyzed nine randomized controlled studies comparing subacromial corticosteroid injection with placebo. One study demonstrated significant pain relief and two studies showed an increased range of motion in the injection group. No significant complications were identified. In a study comparing a corticosteroid injection with a platelet-rich plasma (PRP) injection for subacromial impingement syndrome, the investigators found the Constant score and VAS for pain to be significantly better at both 6 weeks and 6 months in the corticosteroid-treated group. Good patient candidates for a single subacromial corticosteroid injection to achieve pain control are those with significant night pain or patients who will not tolerate phase 1 rehabilitation because of pain.

Although cross-sectional comparison of different shoulder questionnaires can show comparable overall validity and patient acceptability, it is important to include overhead activities since overhead work is an important aspect of her daily work. An additional benefit of the SPADI is that it is responsive to change, quick to complete, and scores are not likely to change in stable subjects (Paul et al 2004).

PRP injections are being utilized for partial rotator cuff tears. There are mixed results in the literature. PEMF therapy effectiveness in mouse models of breast cancer; Mice were divided into four groups (n = 3 each). Group 1, 2, and 3 were exposed to PEMF therapy (1 Hz, 100 mT) daily for 60, 180, or 360 min, respectively, for 4 weeks, while group 4 did not receive PEMF therapy and was used as control. All mice were monitored for tumor growth by body bioluminescence imaging once every 2 to 4 days for 4 weeks. Then, all the mice were sacrificed and skin, liver, lung, and spleen samples were collected for histopathologic analysis. Mice exposed to PEMFs for 60 and 180 min daily showed a 30% and 70% breast tumor reduction, respectively, at week 4. Mice exposed to PEMF for 360 min daily, showed a suppression of tumor growth at week 4. In summary, this study shows that the time of PEMF exposure is critical to determine its effectiveness. Mice exposed for longer duration (360 min daily for 4 weeks) showed a significant reduction in tumor size, due probably to the inhibition of angiogenesis that may suppress the formation of blood vessels in tumor tissues, reducing the tumor growth.

Unfortunately, very few conventional, and even alternative or holistic, doctors know about these devices or this technological area. This is not a subject of mandatory education for doctors. Doctors often learn about these new technologies long after the public does, as has been seen with acupuncture. The process of educating doctors and other non-medical practitioners is growing all the time but will take years. Bergman et al (2004) compared medical care (consisting of oral analgesics or NSAID, education, advice, corticosteroid infiltrations and physical therapy referral for exercise, modalities, massage after 6 weeks) to medical care with up to 6 treatments of thrust and non-thrust manipulative interventions to the ribs and cervical-thoracic spine over 12 weeks in patients with shoulder symptoms and dysfunction of cervico-thoracic spine and adjacent ribs. At 12 weeks, 43% of the manipulation group and 21% of the medical care group reported full recovery. A 17-percentage point difference favouring manipulation still existed at 52 weeks. During intervention and follow-up a consistent between-group difference in severity of the main complaint, shoulder pain and disability, and general health favoured the manual therapy group.
Once it was established that Ca2+ binding to CaM was a potential transduction pathway for PEMFs, the electrochemical model was employed to configure RF signals that would efficiently couple to Ca2+-binding kinetics28 using rate constants, which are well studied for the Ca/CaM system.39 This enabled the diathermy-based PEMF signal to be reconfigured so that its frequency spectrum more closely matched the dielectric properties of Ca2+-binding kinetics at CaM. The result is a PEMF device that uses 100 times less peak power to produce a biologically effective signal dose in the body. Initial confirmation of these predictions of the electrochemical model were reported for the MLCK enzyme assay, neurite outgrowth, and bone repair in a rabbit model.40 All of the limitations of the original diathermy-based devices were therefore addressed, potentially providing the physician with a more versatile and economical tool for postoperative pain and edema management with no known side effects.3
Cognitive functioning after repetitive transcranial magnetic stimulation in patients with cerebrovascular disease without dementia: a pilot study of seven patients; Rektorova; J Neurol Sci. 2005 Mar 15;229-230:157-61 This study aimed to examine whether one session of high frequency repetitive transcranial magnetic stimulation (rTMS) applied over the left dorsolateral prefrontal cortex (DLPFC) would induce any measurable cognitive changes in patients with cerebrovascular disease and mild cognitive deficits. Seven patients with cerebrovascular disease and mild executive dysfunction entered the randomized, controlled, blinded study with a crossover design. rTMS was applied either over the left DLPFC (an active stimulation site) or over the left motor cortex (MC; a control stimulation site) in one session. Each patient participated in both stimulation sessions (days 1 and 4) and the order of stimulation sites (DLPFC or MC) was randomized. A short battery of neuropsychological tests was performed by a blinded psychologist prior to and after each rTMS session. Psychomotor speed, executive function, and memory were evaluated. Results: mild but significant stimulation site-specific effect of rTMS was observed in the Stroop interference results (i.e. improvement) after the stimulation of DLPFC in comparison with the baseline scores (Wilcoxon, Z=-2.03, p=0.04). Patients improved in the digit symbols subtest of the Wechsler adult intelligence scale-revised after both rTMS sessions regardless of the stimulation site.

You may be interested in the work of Body Intelligence/biodynamic craniosacral therapy, Pain is Really Strange (FB and blog site) - although name implies about pain, it's that full mix of what you have been exploring yourself (Steve Haines, craniosacral therapist).
The biophysical mechanism(s) of interaction of PEMF on biologic tissues and the biologic transduction mechanism(s) have been vigorously studied.27 One of the first models created was a linear physicochemical approach,22,24,27,28 in which an electrochemical model of the cell membrane was employed to predict a range of PEMF waveform parameters for which bioeffects might be expected. The most generally accepted biophysical transduction step is ion/ligand binding at cell surfaces and junctions that modulate a cascade of biochemical processes, resulting in the observed physiologic effect.25,29–31 A unifying biophysical mechanism that could explain the vast range of reported results and allow predictions of which PEMF signals and exposures are likely to induce a clinically meaningful physiologic effect has been proposed.22,28 The general application of this approach led to the BGS signal in use today. However, that signal is often only marginally effective because further dose quantification needed specific knowledge of the ion, the target site, its binding kinetics, and the cascade involved.
My experience as a physio in the community has involved lots of work with Parkinson's and increasingly I see the effects of stress and the social engagement system being critical to understanding and improving movement, and in the last 3 years have done much more work with body awareness during activity, whether it be gaining flexibility or strength or balance. The toughest part is engagement especially when the general physio community is not promoting the same message. As you're website implies, it requires such a commitment to your well-being. The pooled evidence suggests that rTMS improves upper limb function in the short-term, walking performance and Unified Parkinson’s Disease rating Scale (UPDRS III) in the short- and long-terms in PD sufferers.
