Exercise therapy interventions for patients with impingement are intended to restore the frontal and transverse plane glenohumeral force couples and normalize scapular motion. Generally they consist of progressive resistive exercises for the rotator cuff and scapular muscles and stretching of tight structures but they should also address the motor control deficits identified in patients with impingement. More detail on shoulder exercises is provided in Chapters 21 and 22. Exercise interventions have been supported in a number of recent randomized trials (Werner et al 2002, Walther et al 2004, Lombardi et al 2008) and systematic literature reviews for producing improvements in both pain and function (Green et al 2003, Trampas & Kitsios 2006, Faber et al 2006). In a Cochrane review (Green et al 2003), exercise was noted as effective in terms of short-term recovery in rotator cuff disease (RR 7.74; 95% CI 1.97–30.32) and for longer-term benefit with regard to function (RR 2.45; 95% CI 1.24–4.86). It should be noted that in patients with Neer stage I–II impingement there are no significant between-group differences (at 6 and 12 weeks) with regard to pain and function for patients treated with a supervised exercise programme or a home programme in which they are instructed by a physical therapist (Werner et al 2002, Walther et al 2004).

PEMF (Pulsed Electromagnetic Field) therapy has been used for centuries in various forms and was researched notably by Nikola Tesla and NASA. Cognitive functioning after repetitive transcranial magnetic stimulation in patients with cerebrovascular disease without dementia: a pilot study of seven patients; Rektorova; J Neurol Sci. 2005 Mar 15;229-230:157-61 This study aimed to examine whether one session of high frequency repetitive transcranial magnetic stimulation (rTMS) applied over the left dorsolateral prefrontal cortex (DLPFC) would induce any measurable cognitive changes in patients with cerebrovascular disease and mild cognitive deficits. Seven patients with cerebrovascular disease and mild executive dysfunction entered the randomized, controlled, blinded study with a crossover design. rTMS was applied either over the left DLPFC (an active stimulation site) or over the left motor cortex (MC; a control stimulation site) in one session. Each patient participated in both stimulation sessions (days 1 and 4) and the order of stimulation sites (DLPFC or MC) was randomized. A short battery of neuropsychological tests was performed by a blinded psychologist prior to and after each rTMS session. Psychomotor speed, executive function, and memory were evaluated. Results: mild but significant stimulation site-specific effect of rTMS was observed in the Stroop interference results (i.e. improvement) after the stimulation of DLPFC in comparison with the baseline scores (Wilcoxon, Z=-2.03, p=0.04). Patients improved in the digit symbols subtest of the Wechsler adult intelligence scale-revised after both rTMS sessions regardless of the stimulation site.
So now I can return to the question I posed at the beginning of this article, “Is Pulsed Electromagnetic Field Therapy dangerous?” In short, No. From all the evidence I have seen and read, there seems to be no immediate damage caused by PEMF. If in fact, this is just an artificially recreated version of the Earths own Pulsed Electromagnetic Field, then I can understand why we aren’t being damaged by it; after all we did evolve under its effects.

The value of pulsed electromagnetic field therapy has been shown to cover a wide range of conditions, with well documented trials carried out by hospitals, rheumatologists, physiotherapists, and neurologists.
Considering the role of thoracic flexion on scapulo-thoracic motion, education with regard to appropriate posture seems an obvious component of patient education. Bullock et al (2005) noted a significant increase in patients with impingement for shoulder flexion range although not pain intensity with erect as compared to slouched sitting posture. Visual, manual, and verbal feedback combined with education on faulty movement patterns provided significantly decreased electromyographic activity in the upper and middle trapezius, infraspinatus, serratus anterior, and anterior and middle deltoid muscles of patients with impingement immediately and 24 hours after movement training, whereas trunk, shoulder, and clavicular kinematics improved during and immediately after training, especially in the subset of patients with elevated clavicular position supporting the role of educating patients on correct movement patterns (Roy et al 2009).
Using outcome measures such as the Upper Extremity Functional Index (Stratford et al 2001) or the Croft Disability Questionnaire (Croft et al 1994) do not measure pain and disability associated with overhead activities as the SPADI does. Although cross-sectional comparison of different shoulder questionnaires can show comparable overall validity and patient acceptability, it is important to include overhead activities since overhead work is an important aspect of her daily work. An additional benefit of the SPADI is that it is responsive to change, quick to complete, and scores are not likely to change in stable subjects (Paul et al 2004).

Having experienced an MRS 2000 and Bemer, I wanted a mat and thought I was getting such a good deal with the OMI. I just want to share my disappointment so that you may benefit from my leaning. First, there is a noise that emanates from the mat and PEMF Supply will tell you that his is normal. Well it isn't with the MRS or Bemer. The noise sounds something like what you might imagine electricity running through a coil would sound. It belied any relaxation for me. Secondly, you will pay at least $225 (my loss in simply returning the system was $374.37) to return it. If you feel you have a faulty mat, you will pay $50 to send it to them to look at and another $50 to send it back to you. For a new product, I feel that is very poor customer service. In light of these aspects, I am not able to recommend the OMI system. If you still want to buy it, I would advise buying from Amazon since they have a return policy.

Pour établir ma progression, j'avais regardé la progression des fichiers PEMF pour calquer au mieux.
