There is currently a lack of guidelines based on randomized, prospective studies to aid the clinician treating partial rotator cuff tears and contusions. Also, most of the available studies lack adequate statistical power. The results of nonoperative management of partial-thickness tears are largely unknown because there are no long-term follow-up studies using a standardized treatment protocol. Nonsurgical treatment is still regarded as the initial management step. The goal of treatment in athletes with a partial rotator cuff tear is to eliminate pain and restore function. Treatment of the athlete with a rotator cuff contusion has the same objective. The goals could evolve if biologic interventions are developed that lead to a true healing response (Ferhat et al, 2016).
The basic science work accomplished to date provides strong support for the proposal that modulation of Ca2+ binding to CaM, upon a transient increase in intracellular calcium when homeostasis is interrupted,48 is an important PEMF transduction pathway. It is likely that the disruption of the tightly regulated Ca2+ balance in cells is the natural signal to provoke the endogenous tissue repair and regeneration mechanism, hence the apparent simple acceleration of normal healing activity by targeted PEMF signals. Ca/CaM catalyzes eNOS, which allows the PEMF signal to modulate the release of NO from eNOS and potentially affect the entire tissue repair pathway, from pain and edema to angiogenesis, bone and tissue regeneration, and other regenerative actions. PEMF signals configured to target the Ca/CaM pathway have been applied to rat tendon and wound healing.49,50 In both studies, tendon and wound healing rates were seen to significantly increase by 59% ± 4% (Figure 1, A) and 69% ± 5% (Figure 1, B) in PEMF-treated animals.

Back in 2002, a pilot study was carried out to objectively assess the effectiveness of EarthPulse. In the study, some 11 patients suffering from chronic pain for more than two years were placed on a PEMF therapy in order to determine how EarthPulse influences sleep patterns and pain reception. After a few months of treatment, 7 of the 11 patients, having been subjectively evaluated by Visual analog pain scales, reported significant alleviation of pain. I know many people with PD have similar problems around food, digestion and medication ineffectiveness.  The reason for this, I believe, is in the role of the primitive Dorsal (Vegetative) branch of the Vagus Nerve in digestion, the specific part of the Nervous System also responsible for immobization/freeze responses in humans. Essentially, when a lot of food hits the digestive tract, this can send the enteric nervous system of people with PD into shock, activating the Dorsal Vagus, but in a way which then also inhibits the Ventral (Smart) Vagus Branch responsible for relaxed digestion. The day before my wedding Dr. Pacelli performed a second examination with digital x-rays to the lower leg and right ankle. It was a fantastic day for the brake was almost completely healed. PEMFs have been in clinical use for generations. For most of that time, however, PEMFs have been relegated to second or even third tier status, with some ardent supporters, a number of skeptics, and most clinicians and patients simply unaware of their benefits. Without substantive information about a mechanism of action and frequently being the subject of overzealous marketing and inflated claims, PEMF devices lacked credibility. When the knowledge base in basic sciences allowed for the critical examination of PEMF in the laboratory and provided techniques for both targeting and engineering, the system advanced and the many effects of PEMF signals could be rationalized within at least 1 biologic cascade—one that is dependent on an electrochemical process and can be affected by exogenous signals. As this body of evidence grows and clinical experience widens, the gaps in the current knowledge (especially concerning optimal treatment regimens for specific conditions) will be filled. At the same time, we anticipate that improved signals and products that are more effective and more ergonomically designed will be developed, and that other electrochemical pathways will be targeted for additional indications. This may finally be the century of electrotherapy. In the meantime, plastic surgeons have at hand a powerful tool for the adjunctive management of postoperative pain and edema and wound repair. Cancer is seldom found isolated from other conditions; many of our patients have multiple diagnoses, coupled with their cancer challenge. I have worked with a number of Parkinson's clients very effectively, but - as is often the case with 'complementary' approach, the challenge is in embracing quite a different way of thinking - and the medication/grip of disease/anxiety and stress are powerful and seductive hooks. The gentlemen I am looking into gut health for has found after a couple of our sessions, but not all the time, he is able to play piano after 9 years of his tremor being too disruptive. CONCLUSIONS: This review shows that plastic surgeons have at hand a powerful tool with no known side effects for the adjunctive, noninvasive, nonpharmacologic management of postoperative pain and edema. Given the recent rapid advances in development of portable and economical PEMF devices, what has been of most significance to the plastic surgeon is the laboratory and clinical confirmation of decreased pain and swelling following injury or surgery.
Exposure to a specific pulsed electromagnetic field (PEMF) has been shown to produce analgesic (antinociceptive) effects in many organisms. In a randomized, double-blind, sham-controlled clinical trial, patients with either chronic generalized pain from fibromyalgia (FM) or chronic localized musculoskeletal or inflammatory pain were exposed to a PEMF (400 μT) through a portable device fitted to their head during twice-daily 40 min treatments over seven days. The effect of this PEMF on pain reduction was recorded using a visual analogue scale. A differential effect of PEMF over sham treatment was noticed in patients with FM, which approached statistical significance (P=0.06) despite low numbers (n=17); this effect was not evident in those without FM (P=0.93; n=15). PEMF may be a novel, safe and effective therapeutic tool for use in at least certain subsets of patients with chronic, nonmalignant pain.
In an episode of the Bulletproof Radio podcast, PEMF expert Dr. Gary Ryan, known as “The Energy Doctor,” explains, “Based on a lot of research that was done at Yale, it is apparent that just about any pathology in the body is preceded by a drop in cell charge. Now we have technology that will reach down to the level of a cell that has lost charge and, due to the high intensity of the pulse, bring that pulse back to normal or a more normal situation, which allows it to replicate and produce a more normal cell.”

As we can see, long-term pulsed magnetic field therapy has been proven to be safe, so you should be able to proceed with using it without any issues, especially if it makes you feel better. Remember, listen to your body and when in doubt always consult a professional who knows about using PEMF treatment. Approaching professionals who do not have experience with a particular type of treatment will almost always be counter-productive as they will not be able to guide you with a protocol they don’t understand. With regard to medical management, Green et al (2003) reported that for rotator cuff disease, corticosteroid injections might at times be superior to physical therapy. Buchbinder et al (2003) noted that for rotator cuff disease, subacromial steroid injection demonstrated a small benefit over placebo in some trials. Pooled results of three trials showed no benefit of subacromial steroid injection over NSAIDs. In the context of surgery it should be noted that no significant differences have been reported in outcome between arthroscopic and open subacromial decompression, although four trials did report earlier recovery with arthroscopic decompression (Coghlan et al 2008).
