"In October 2008 the Food and Drug Administration approved the use of PEMF therapy for treatment of major depressive disorder in PD patients who failed to achieve satisfactory improvement from very high dosages of antidepressant medications. Several studies reported PEMF therapy improved cognitive functions and motor symptoms. For example, an investigation involving three elderly PD patients with cognitive impairment assessed the effect of PEMF therapy on a disorder of the body image in which the patient perceives a part or parts of his body as disproportionately large. After receiving PEMF therapy, PD patients’ drawings showed reversal of macrosomatognosia (assessed by Draw-a-Person test) with reduction of the right parietal lobe dysfunction."

Exercise therapy interventions for patients with impingement are intended to restore the frontal and transverse plane glenohumeral force couples and normalize scapular motion. Generally they consist of progressive resistive exercises for the rotator cuff and scapular muscles and stretching of tight structures but they should also address the motor control deficits identified in patients with impingement. More detail on shoulder exercises is provided in Chapters 21 and 22. Exercise interventions have been supported in a number of recent randomized trials (Werner et al 2002, Walther et al 2004, Lombardi et al 2008) and systematic literature reviews for producing improvements in both pain and function (Green et al 2003, Trampas & Kitsios 2006, Faber et al 2006). In a Cochrane review (Green et al 2003), exercise was noted as effective in terms of short-term recovery in rotator cuff disease (RR 7.74; 95% CI 1.97–30.32) and for longer-term benefit with regard to function (RR 2.45; 95% CI 1.24–4.86). It should be noted that in patients with Neer stage I–II impingement there are no significant between-group differences (at 6 and 12 weeks) with regard to pain and function for patients treated with a supervised exercise programme or a home programme in which they are instructed by a physical therapist (Werner et al 2002, Walther et al 2004).
Many PEMF mats come with other therapies programmed into the mat. If your mat has more than one therapy, try not to enable all the therapies at once. Instead, enable 1-2 therapies every 3 days (ex. Adjuncts in diminishing the chemical reaction of the inflammatory process are rest, therapeutic modalities, and nonsteroidal anti-inflammatory agents. When the inflammatory mechanism is inhibited, the patient should experience an associated decrease in pain and swelling. Success in achieving these goals has been accomplished through an array of modalities to include laser, microcurrent, pulsed electromagnetic field therapy, iontophoresis, and phonophoresis.117 My (MAK) anecdotal choice of modalities to treat the acute inflamed shoulder include cryotherapy and low-frequency transcutaneous electrical nerve stimulation (TENS).

There followed the development of inductively coupled, externally applied electromagnetic field modalities to affect bone repair.22,23 Development of the bone growth stimulator (BGS) signal did not follow from diathermy considerations, but rather from the general electrochemical models developed by one of the authors (AAP).24 As will be seen, although this approach resulted in an effective BGS signal, that signal was not specifically configured for what is now commonly considered to be the PEMF transduction pathway. Nonetheless, a multitude of studies have shown the BGS signal to have sufficient biologic effect to modulate growth factor release.25 Therapeutic uses of these technologies in orthopedics have led to clinical applications approved by regulatory bodies worldwide for the adjunctive treatment of recalcitrant fractures and spine fusion.1,26 Several reports have suggested that the overall success rate of BGS is not significantly different from that of the first bone graft,1 which is a significant benefit for the patient and the health care system.

I know many people with PD have similar problems around food, digestion and medication ineffectiveness.  The reason for this, I believe, is in the role of the primitive Dorsal (Vegetative) branch of the Vagus Nerve in digestion, the specific part of the Nervous System also responsible for immobization/freeze responses in humans. Essentially, when a lot of food hits the digestive tract, this can send the enteric nervous system of people with PD into shock, activating the Dorsal Vagus, but in a way which then also inhibits the Ventral (Smart) Vagus Branch responsible for relaxed digestion. Several mechanisms of PEMF therapy have been elucidated with regards to cancer. These studies have shown that PEMF therapy may exert proliferative inhibition and mitotic spindle disruption, block the development of neovascularization (blood supply) required for tumor growth and exacerbate an inherent or induced genetic instability by reducing the stringency of the late-cycle (G2) checkpoint. PEMF therapy also modulates gene expression and protein synthesis, interacting with specific DNA sequences within gene promoter regions. PEMFs have also an immunomodulatory effect, as supported by in vivo evidence showing an increase in tumor necrosis factor alpha levels that induce an anti-tumoral response.
A systematic review aimed to examine the efficacy of rTMS on improving physical function and motor signs over the short- and long-terms in people with PD; Effect of Repetitive Transcranial Magnetic Stimulation on Physical Function and Motor Signs in Parkinson's Disease: A Systematic Review and Meta-Analysis; Chung CL; Brain Stimul. 2016 Jul-Aug;9(4):475-87. Twenty-two trials comprising 555 people with PD were included in the review. The pooled evidence suggests that rTMS improves upper limb function in the short-term, walking performance and Unified Parkinson’s Disease rating Scale (UPDRS III) in the short- and long-terms in PD sufferers.
It is important to note that PEMF effects are immediate and are not limited by pharmacokinetics because the induced currents are instantaneously present when the coil applicator is transmitting into the affected area. For example, studies designed to assess PEMF effects on pain and edema in a carrageenan rat hind paw model have reported a 100% inhibition of pain and a 50% reduction of edema in treated animals over a time span of 225 minutes62 compared with aspirin or nitroaspirin, which only caused about 50% pain inhibition at 200 minutes, using maximum dose in the same model.63 It is also important to note that resting cells (in homeostasis), in which there is no transient increase in cytosolic free Ca2+, do not appear to respond to PEMF, providing one explanation for the reports of no known side effects from PEMFs since the clearance of BGS devices in 1979.

The first study utilizing PEMF therapy was conducted by Barbault and coworkers who hypothesized that a combination of defined tumor-specific frequencies, may display therapeutic effectiveness for localized treatment of tumors. They identified a total of 1524 tumor-specific frequencies, ranging from 0.1 to 114 kHz, consisting in the measurement of variations in skin electrical resistance, pulse amplitude, and blood pressure in 163 patients affected by different types of cancer including brain tumors, colorectal cancer, hepatocellular carcinoma, pancreatic, colorectal, ovarian, breast, prostate, lung, thyroid, and bladder cancer and exposed to the radiofrequency system.


The charge of the cell membrane is affected by magnetic fields, causing the membrane channels to open up. These channels are similar to the windows and doors of a house. When the cell channels are opened, nutrients enter the cell more easily and the cellular waste is more easily removed, helping to rebalance and restore optimum function of the cell.

Exposure to a specific pulsed electromagnetic field (PEMF) has been shown to produce analgesic (antinociceptive) effects in many organisms. In a randomized, double-blind, sham-controlled clinical trial, patients with either chronic generalized pain from fibromyalgia (FM) or chronic localized musculoskeletal or inflammatory pain were exposed to a PEMF (400 μT) through a portable device fitted to their head during twice-daily 40 min treatments over seven days. The effect of this PEMF on pain reduction was recorded using a visual analogue scale. A differential effect of PEMF over sham treatment was noticed in patients with FM, which approached statistical significance (P=0.06) despite low numbers (n=17); this effect was not evident in those without FM (P=0.93; n=15). PEMF may be a novel, safe and effective therapeutic tool for use in at least certain subsets of patients with chronic, nonmalignant pain.
The feasibility of PEMF therapy for treatment of hepatocellular carcinoma (HCC) has also been investigated in a single-group, open-label, phase I/II clinical study. Forty-one patients with advanced HCC received very low levels of PEMFs modulated at HCC-specific frequencies (100 Hz–21 kHz) and received three-daily 60 min outpatient treatments. No adverse reactions were observed during PEMF treatment. Five patients reported complete disappearance and two patients reported decrease in pain shortly after beginning of treatment. Four patients showed a partial response to treatment, while 16 patients (39%) had stable disease for more than 12 weeks. This study shows that PEMF therapy provides a safe and well-tolerated treatment, as well as evidence of antineoplastic effects in patients with HCC.

Determining whether the injury is traumatic or is related to overuse is the first step to frame the treatment strategies. Pulsed electromagnetic field therapy, with other less commonly used interventions, was not included in this list, and its evidence for efficacy was therefore not assessed. However, as emphasised by Dr Pfeiffer, an evidence based evaluation of all other interventions would be of interest and could be considered for inclusion in the next round of evidence based guidelines.
