I know many people with PD have similar problems around food, digestion and medication ineffectiveness. The reason for this, I believe, is in the role of the primitive Dorsal (Vegetative) branch of the Vagus Nerve in digestion, the specific part of the Nervous System also responsible for immobization/freeze responses in humans. Essentially, when a lot of food hits the digestive tract, this can send the enteric nervous system of people with PD into shock, activating the Dorsal Vagus, but in a way which then also inhibits the Ventral (Smart) Vagus Branch responsible for relaxed digestion. While it’s true not that all pain could possibly healed by a single method, as the causes of the pain matter most and may not be treatable by PEMF, which targets to heal the cause of the pain as well. For example, PEMF is not effective in pathogenic environments as the microorganisms will also find PEMF nutritious and thrive in it. In such pathogenic environment, such as in case of Lymes Disease, electric zapper devices might be the best electrotherapy mode rather than PEMF. Although I am certified in cardiology and gastroenterology, a significant portion of my practice is now devoted to exploring how PST achieves its benefits.10 I have treated 1000 patients, most of whom had knee OA, with very gratifying results similar to those reported in the literature. The panel cited two prior efforts to establish guidelines for treating knee OA, emphasising that these “primarily represent consensus statements from expert panels” and “The type and strength of evidence to support such guidelines remain unclear.” Their stated objective, therefore, was to “develop guidelines relating to clinical issues in OA management, and to indicate clearly the level of evidence to support individual statements”. However, electromagnetic therapy approaches were again omitted, though at least one of the members is quite familiar with PST. The reason for this exclusion is not clear and I believe that your readership deserves to be aware of this extremely safe and effective option.

PRP injections are being utilized for partial rotator cuff tears. There are mixed results in the literature. A randomized controlled trial (level I evidence) with a 1-year follow-up demonstrated no benefit from a single PRP injection in comparison with placebo (Kesikburun et al, 2013). Biologic type injections may have a role in the treatment of partial rotator cuff tears; however, the exact platelet count and leukocyte concentration in PRP for optimal growth factor activity has yet to be definitively proven in the literature.
Determining whether the injury is traumatic or is related to overuse is the first step to frame the treatment strategies. In the athlete with a trauma history, the initial goal is to control the pain and to restore full shoulder/scapula mobility and strength. The treatment for a partial rotator cuff tear in the overhead throwing athlete centers on the underlying deficiencies seen, such as loss of shoulder motion, scapula dyskinesia, and rotator cuff weakness. Mechanical flaws in the throwing or overhead motion may also need to be corrected.

As with back pain each of the possible contributing factors need to be examined and included in the treatment plan as appropriate (Kent et al 2005). This means that instead of lumping groups of symptoms together (e.g. rotator cuff symptoms) it has been suggested to split and recognise factors that cause laxity, impingement and/or lesion and provide treatment as the clinician sees fit. The most recent studies of the PEMF transduction pathway have concentrated upon the Ca/CaM-dependent nitric oxide (NO) cascades. However, those linkages were dependent on the discovery that NO is a signaling molecule.41 NO is synthesized via nitric oxide synthase (NOS), that has several different isoforms.42 When injury occurs, large amounts of NO are produced by long-lived inducible nitric oxide synthase (iNOS). In this cascade, tissue levels of NO persist and the prolonged presence of this free radical is proinflammatory,43 which accounts for the leaky blood vessels associated with pain and swelling.44 In contrast, the endothelial and neuronal nitric oxide synthase isoforms (eNOS and nNOS, respectively) produce NO in short bursts that can immediately relax blood and lymph vessels.45,46 These short bursts of NO also lead to the production of cyclic guanosine monophosphate, which in turn drives growth factor production.47 Interestingly, iNOS is not dependent on CaM,43 while the constitutive or cNOS (eNOS or nNOS) cascade is dependent on the binding of Ca/CaM. Therapies that could accelerate Ca/CaM binding, therefore, should impact all phases of tissue repair, from initial pain and swelling to blood vessel growth, tissue regeneration, and remodeling.42
