Having experienced an MRS 2000 and Bemer, I wanted a mat and thought I was getting such a good deal with the OMI. I just want to share my disappointment so that you may benefit from my leaning. First, there is a noise that emanates from the mat and PEMF Supply will tell you that his is normal. Well it isn't with the MRS or Bemer. The noise sounds something like what you might imagine electricity running through a coil would sound. It belied any relaxation for me. Secondly, you will pay at least $225 (my loss in simply returning the system was $374.37) to return it. If you feel you have a faulty mat, you will pay $50 to send it to them to look at and another $50 to send it back to you. For a new product, I feel that is very poor customer service. In light of these aspects, I am not able to recommend the OMI system. If you still want to buy it, I would advise buying from Amazon since they have a return policy.

The most recent studies of the PEMF transduction pathway have concentrated upon the Ca/CaM-dependent nitric oxide (NO) cascades. It is within this system that the effectiveness of PEMF is now understood to function. However, those linkages were dependent on the discovery that NO is a signaling molecule.41 NO is synthesized via nitric oxide synthase (NOS), that has several different isoforms.42 When injury occurs, large amounts of NO are produced by long-lived inducible nitric oxide synthase (iNOS). In this cascade, tissue levels of NO persist and the prolonged presence of this free radical is proinflammatory,43 which accounts for the leaky blood vessels associated with pain and swelling.44 In contrast, the endothelial and neuronal nitric oxide synthase isoforms (eNOS and nNOS, respectively) produce NO in short bursts that can immediately relax blood and lymph vessels.45,46 These short bursts of NO also lead to the production of cyclic guanosine monophosphate, which in turn drives growth factor production.47 Interestingly, iNOS is not dependent on CaM,43 while the constitutive or cNOS (eNOS or nNOS) cascade is dependent on the binding of Ca/CaM. Therapies that could accelerate Ca/CaM binding, therefore, should impact all phases of tissue repair, from initial pain and swelling to blood vessel growth, tissue regeneration, and remodeling.42

There followed the development of inductively coupled, externally applied electromagnetic field modalities to affect bone repair.22,23 Development of the bone growth stimulator (BGS) signal did not follow from diathermy considerations, but rather from the general electrochemical models developed by one of the authors (AAP).24 As will be seen, although this approach resulted in an effective BGS signal, that signal was not specifically configured for what is now commonly considered to be the PEMF transduction pathway. Nonetheless, a multitude of studies have shown the BGS signal to have sufficient biologic effect to modulate growth factor release.25 Therapeutic uses of these technologies in orthopedics have led to clinical applications approved by regulatory bodies worldwide for the adjunctive treatment of recalcitrant fractures and spine fusion.1,26 Several reports have suggested that the overall success rate of BGS is not significantly different from that of the first bone graft,1 which is a significant benefit for the patient and the health care system.
Considered together, all available evidence provides strong support for Ca/CaM-dependent transient NO production as an important PEMF transduction pathway for tissue repair. One of the authors (AAP) has recently proposed the PEMF mechanism as a working model for PEMF therapeutics (Figure 4).61 This mechanism suggests that the primary effect of clinically relevant PEMF signals is to increase the rate of Ca2+ binding to CaM, which then catalyzes cNOS (eg., eNOS), producing an immediate (within seconds) production of NO, which can orchestrate an antiinflammatory response via increased blood and lymph flow. NO, in turn, regulates cGMP production (within minutes), which cascades to the appropriate growth factor release dependent on the stage of healing (eg., FGF-2 for angiogenesis).

With regard to medical management, Green et al (2003) reported that for rotator cuff disease, corticosteroid injections might at times be superior to physical therapy. Buchbinder et al (2003) noted that for rotator cuff disease, subacromial steroid injection demonstrated a small benefit over placebo in some trials. Pooled results of three trials showed no benefit of subacromial steroid injection over NSAIDs. In the context of surgery it should be noted that no significant differences have been reported in outcome between arthroscopic and open subacromial decompression, although four trials did report earlier recovery with arthroscopic decompression (Coghlan et al 2008).

TENS stands for Transcutaneous Electrical Nerve Stimulation which is a non-invasive drug free method of pain relief for both symptomatic and chronic pain. Transcutaneous means across the skin. A TENS machine relieves pain by sending tiny electrical impulses through electrodes placed on or near the area of pain or pressure point to underlying nerve fibres.

Considering the role of thoracic flexion on scapulo-thoracic motion, education with regard to appropriate posture seems an obvious component of patient education. Bullock et al (2005) noted a significant increase in patients with impingement for shoulder flexion range although not pain intensity with erect as compared to slouched sitting posture. Visual, manual, and verbal feedback combined with education on faulty movement patterns provided significantly decreased electromyographic activity in the upper and middle trapezius, infraspinatus, serratus anterior, and anterior and middle deltoid muscles of patients with impingement immediately and 24 hours after movement training, whereas trunk, shoulder, and clavicular kinematics improved during and immediately after training, especially in the subset of patients with elevated clavicular position supporting the role of educating patients on correct movement patterns (Roy et al 2009).

CONCLUSIONS: This review shows that plastic surgeons have at hand a powerful tool with no known side effects for the adjunctive, noninvasive, nonpharmacologic management of postoperative pain and edema. Given the recent rapid advances in development of portable and economical PEMF devices, what has been of most significance to the plastic surgeon is the laboratory and clinical confirmation of decreased pain and swelling following injury or surgery.

I bought a mat because I’ve read so much research over the last few years on Beamer therapy. I had to protruding discs in my lower back years ago and my job requires me to drive a lot . So I usually have a lot of stiffness & back pain. I used pemf about 7 times now and my back pain is so relieved. I can go hours of driving without as much back pain. When I wake up in the morning I’m not as stiff and I have seen ALIT of energy. I have a friend who uses mine for LUPUS and they have seen nothing but more energy. I definitely would recommend this, and will be purchasing more.
